Admission Note: Level Up Healthcare Academy reserves the right to accept or reject all applicants. If an applicant does not meet admission requirements, or is rejected for training, the applicant will be notified in writing, and all previous obligations within this contract will be void. Admission is at the discretion of the director. All monies paid by the applicant will be refunded according to the refund policy within 40 days minus the SLED application fee and an administrative fee of $100.00 and there will be no future liability. I further understand that the deposit holds my seat only until such time a decision is made by the director, and I will be notified asap within 7 days in writing of their decision.

Please Note: If you are unable to attend your scheduled session, you may request to reschedule for a different session, subject to availability. However, please be aware that no refunds will be issued for missed sessions.

Admission & Enrollment Requirements

Students must be at least 16 yr. old, have proof of immunizations, drug testing and background checks. Students must also adhere to all program rules, including attending all classes and clinical. Failure to comply with these requirements will result in dismissal from the program. The public expects and deserves highly qualified professional nurse aides to care for them and their families!
